Community Re-integration

The Community Reintegration program is part of the Rehabilitation Service aimed at providing the support and services necessary for our clients to achieve their optimal level of psycho- social and occupational functioning.

This provides a continuum of services from the inpatient into the community, providing a platform that helps our clients to transition towards their long-term goals such as returning to work, going back to school or pursuing vocational training

It is integrated into the day to day care during the rehabilitation process. Individuals work with our multidisciplinary teams depending on their individual needs on a one-on-one, group and family sessions modeled around creating each person’s unique abilities including:

1. Problem solving

2. Communication skills

3. Coping mechanisms

4. Independent living skills

5. Social skills

These skills reinforce each one’s capacity for independence and to increase ability for social interaction.  On discharge from the formal program, individuals continue to participate in weekly individual and group meetings with the aim of transitioning skills learnt during the rehabilitation process into their day to day lives in the community.

Treatment modalities employed include:

1. Individual and group therapy

2. Family therapy

3. Social and recreational activities

4. Skills training

5. Linkage to other agencies and resources as needed
